Received file number .How to add spouse to the application??

Hello guys, I need some information regarding adding spouse to my application. I just received my file number. Could you please tell me how to proceed?. What all documents needed to be filled.Would be happy for your responses.

Contact your visa office and inform them that you have a change in family status, you have gotten married and want to add your wife to your application. They will provide you with instructions...

Does the last name on the Spouse's passport has to be changed in this case ?

@Torap..I don't think so..Just the forms related to spouse,marriage record, 4photos, Birth certificate, Passport copies, $550 fee
