I submitted my application in January with my upfront medical which has six month left to expire. three days after submitting the application my medical was passed. since then no update. my medical will expire in coming June and today I received a BVL and eligibility started. any idea what is happening

It's very tough to understand IRCC's activities these days. I just hope you get a good result soon.
I am in the same situation. Medical passed on 20th May.... received bvl on 27th May and now the medical is expired. They haven't requested any new medical tests. Please help.. Do I go ahead and get Upfront medical and send it to them or wait for them to request?
You can wait for request since your application is still in progress. You cannot travel to Canada without valid medicals so you would have to do it eventually. The only question is when.
