Schedule A: A separate and newly completed Schedule A Background Declaration form
(IMM 5669) bearing an original signature. An updated Schedule A is required. Ensure Question
6 is complete and please leave no gaps.

Just provide CIC what they are asking for. It is pretty generic so many applicants gets the same request. Do not worry your file is in good shape. Best of luck!
I did sent my Schedule A via postal mail along with the PCC. In your case please follow their instructions you might have to send online. Double check on mycic account.
