received 2nd aor in oct 2010....estauts is received by visa office?

I have the a problem, I received 2nd aor in oct 2010..... but estatus is received by visa office....does it mean that once we received 2nd AOR means the status should change to in process....?i was under the impression that the status changes to in process when they are ready to send the request for medicals?

pls advice on the same.
