Were the forms of OHIP ? Yes

the way we had filled the B4 as per list was not efficient for him- We had created 5 Categories. then listed items we are carrying and to follow
then listed the total of all of them together in each category.
He just created 2 categories - With us and to follow i believe it is some what up to the officer

How did he want it ? (Categorization...) - Answered
