1. As per my knowledge, while entering the name of your spouse, enter as per passport, don't copy from any other docs. Because my educational documents , birth certificate everything was just my name and initial, not as per my passport but nothing happened in my application till now. So please don't tick 'YES'. I am filling up 'Generic Form' to send along with my spouse's application as a dependent (the VO never asked me to send any updated forms for me though). Under the personal details for the dependent it is asked "Have you ever used any other name (e.g.Nickname, maiden name,alias etc.)". My spouse used to use only his first name till 2012 and in 2012 he changed his name to add his surname. Thus all of his educational documents show his first name but passport (as it was made after name change) shows the full name with surname.
Should we check 'Yes' in the question? If yes, then do we need to write only given name in the place of 'given name' and leave the 'surname' space empty or write 'NA' in 'surname' place?
Also another question. Am I doing right thing by sending the updated forms 'Generic Form' and 'Additional Family Information Form' even though VO only asked for the forms for my husband and did not ask any form for me?

For the PCC and medicals, i don't think VO have requested you to submit the passport copies because we have already submitted it with the application. Anyway if they have, then you have to give all copies including the blank pages. Please read the request letter. They will mention clearly what they are required.wanderlust7 said:Hello

I just want to ask if I am meant to photocopy each page in my passport even the pages are blank, without any visa or travel stamps ?
Or should i only photocopy the pages that have stamps and visa on ?
My passport is jumbo size and still fairly new, so many unused pages. should I still photocopy those pages ?

I tried to get extension for submitting PCC but the WVO mailbox was full and my email couldnt get delivered. You can try sending email and if u r lucky they may reply you back. But my suggestion is try to get your PCC ASAP and submit it within a given time periodhardik_shah said:Anyone applied for time extension to submit PCC at WVO? i just want to know whether they give some additional time or not, I have everything done, except one PCC.
