This is fine, submit those- I assume ur Hospital is E-medical enabled, and it will be uploading ur Medical results, in such cases even proof submission is optional but better to submit ur hospital receipts
4. Do i need to send the original medical receipt or its copy ? I do not receive any emedical form . I have done it from ruby hall pune .
Copy is fine (again assuming its an E-medical center)
5.Do I need to attach a covering letter other than that received with medical request ? If so any samples for reference ?
