Where you will be sending these docs to NDVO or Colombo?
It is advisable to put a cover letter and write your Application/UCI no on all the pages. The sample format for the cover letter is as under:

I would like to inform you that, I have received a Medicals notification e-mail from New Delhi Visa office
on date xxxxxxxxxxx for me, my spouse and my Son. So, accordingly I have completed my medicals and
want to update on the same by sending you the below listed documents along with RPRF Fee as
requested.
