Just wanted to confirm that if I am in Canada with a work permit for 3 years and I need a visa for re-entry, what is the application # (i.e.. the IMM application number) that I need to fill in.

What method would be best, on-line or paper application.

I would recomend online, it is quicker. Use the tool to get the most updated forms: http://www.cic.gc.ca/english/immigrate/eligibility.asp
