As usual, CIC makes things very confusing... Do i include 2 forms or just 1?
Logic tells me that 1 form with both of our signature should be enough, listing my self in section A and my wife in section B, signing, scanning and sending off for processing. On the other hand the website mentioned 1 form for each person
Conclusion: I will send 1 form and see what happens

Secion B, the spouse (for family sponsorships, this will be the sponsoring spouse or partner) fills in and signs also. There are people who claim not to have filled this out and things turn out ok...we just filled it in and sign it just in case. The logic for filling this out is the the case notes also contain information about the spouse. Multiple roles for the spouse here: spouse and also sponsor at the same time.
Section C, the person who should receive the GCMS notes. This would be the sponsor (the Canadian citizen or PR who is actually able to apply for the notes).
So B and C would be the same person in your case. Section B would need a signature, while Section C does not.
Yes, only one form is needed.
Do make sure that the both signatures are signed on the same piece of paper. That is, one person has not signed it first and then scanned it, and afterwards the other person prints it out and then signs on top of this printed copy, then scan for upload. Another person in this forum did this, as the spouse was not at the same location. CIC replied that the signature has a problem, and asked for the form to be re-submitted. Doing it this way, it is obvious the signatures are not on the same piece of paper.
