Quick Question Regarding "Fee Payment form" Federal Skilled worker class

hi all, i have got the Band draft with me. now filling the form. I have filled Section "A" and selected the relevant check box on form.

but at the right bottom there is a column for DATE, do i have to mention the date? or it is part of SECTION B

if you r talking about additional family info form then yes, u have to sigh and fill the date it is the part of section A if u read the heading on th box there is n NOTE which says "If no spouse,ex-spouse or comman law partner is listed in section A read and sign"

no no, actually i am talking about the fee payment form. have u seen that form. probably it just made must to be the part of forms to be sent to CIO

i think its part of column B, and there is no space for signatures and date of column A applicants.

hi if i choosed section A, and ian paying through a certified cheque, what should i write under "name of payer" and "address of payer"? my name & address or the Bank's.? please help
