quick question about NS express Entry

Hi everyone ,
quick question , does anybody know what is
"Signed notice re: personal information collection
Signed notice"
&
"Signed declaration
Signed declaration"
when I submit the online application for the NS express entry ,
ask me to upload these 2 documents ,

I got it , if anybody has the same question , while when you do the online application , click print , print out the page 11&12 , sign it and scan it , then upload
