Can anyone answer my questions related to the FD-258 fingerprinting card?

Today I went to the police department at Abu Dhabi (UAE) to get the FD-258 fingerprinting. But when I came back, I noticed that they did not provide ORI # (Originating Agency Identifier) and the OCA #. Are these two pieces of information mandatory for getting the Police Certificate from the US?

Another thing is that the FD-258 form given by the police department is an old version. I checked online and found a newer version. Will the old version a problem?

Any insights on these two questions. Thanks a million!!!

you know you can just fingerprint yourself with the appropriate ink and some practice on other sheets of paper. FBI will accept them, at least they accepted mine.
