Questions regarding medical exam proof, and fingerprints for FBI background...

Hello, I am finally about to send off my application after tedious combing-through and found some things I am having trouble for sure confirming or finding people to help me to confirm.

1) What specific document should I be sending as proof of my medical exam? I only received, initially, a receipt. I asked the office if this would be enough, and they said yes, however after searching through immigration forums I found that many people have had this told to them and it turned out to be false. I went back and requested the initial medical report Section ! with the physician's summary (form IMM 1017 SCL F). It is stamped, signed, and otherwise fully filled out. Is this the form I should be sending? Should I send the receipt as well?

2) My FBI background check was clear, but I only received confirmation from the FBI of that. The letter specifically states they no longer return the actual fingerprint cards themselves due to the risk of identity theft. According to them, they stopped returning them a couple of years ago. The document checklist seems to state that I should have had these returned to me because they have to be sent along with the FBI letter I received; the police department where I had the prints done didn't know the answer to this. Is the FBI's response clearing me enough, or do I need to go have my prints re-done and send them off with the application?

Thank you so much for any help. These are the two areas that are holding us up the most. Ideally I'd like to have this sent out by the beginning of February at the absolute latest.

If you are asking because you want to know, I would hazard a guess here:

1. The receipt IS actually enough for the medical, it does not have to be a form.

2. You don't send your actual fingerprint card to CIC. You only send the results that the FBI sends to you. Send the original one, not a photocopy of the one they send you.

so for the proof of medical examination, the receipt from the doctor is enough?

agreed whats been said so far, for the medical, when you do the exam you dont actaully need anything from the physican. They will send teh results to CIC directly. But so remove confusion, what i recomend, and what i did was to get teh reciept from teh doctor, and just write a one-pager stating you were told the results would be sent directly.

For the FBI, just send them the blue result sheet when the local VO asks for it, or include with application, no need to wait for the police reports to send application in. Also, no need to send fingerprints to CIC (also they never asked for this)
