Questions on sending the application

I have the following questions on sending the final documentation [ Question A ] Do we need to include all the sheets of passport ( particularly the last sheet of "Indian" passport ). Checklist indicated only the fist page and pages showing amendments [ Question B ] Should the passport size pictures need be signed by the photo studio (or just date and address of the business suffice ) [ Question C ] In Document checklist item#16 , asks for TWO self addressed mailing labels Should they be blank address stickers sold at STAPLES OR plain paper with address on it will do ? [ Question D ] Should we staple all the relevant documentation and relevant application forms together (if they are more than ONE page) OR use a heavy duty paper-clip OR send loose sheets in the order of the check list.

First off your post is heavy in colors and very busy. This is one of the reasons you didn't get a reply. Try to divide your questions into paragraphs so the whole chunk looks easier to read.

Now to answer your questions:

A/ No you don't need to include all the sheets. Copies of the first page with details and any pages with stamps and information. You can leave the blank pages aside.

B/ The date on the back of the photographs are important. No need to sign.

C/ Mailing labels can be anything. You can print them off on a regular paper and send them. In my case I forgot to include them and nothing happened.

D/ Do not staple anything, use paper clips for every bunch of papers you're providing.

Please find the answers as below :

A - Just send the first page and any page with name/address/personal info change, you can include last page if you want (optional). refer to check list (http://www.cic.gc.ca/english/pdf/kits/forms/IMM5610E.pdf)

B - 6 photos date stamped by photo studio + for 5 of them put name and date of birth of the person in photo (ref check list)

C - You can send any two labels (better if you buy self adhesive ones), with your address printed on them

D - Do not staple anything (not specified anywhere i think), but try to stick multiple-paged documents together with paper clips

The check list says, leave the 6th photo blank (no date, no stamp of the business nor name and date of birth). I believe it is totally blank

Appendix A: Photo specifications

TAKE THIS SHEET WITH YOU TO THE PHOTOGRAPHER

Immigration photos are not the same as passport photos.
Make sure that you provide the correct number of photos specified in the Checklist.
Notes to the photographer

The photos must:

show a full front view of the person's head and shoulders showing full face centered in the middle of the photograph;
have a plain white background;
be identical (black and white or colour) produced from the same unretouched negative, or exposed simultaneously by a split-image or multi-lens camera.

measure between 25 mm and 30 mm (1″ and 1 1/5″) from chin to crown
have a 35 mm x 45 mm (1 3/8″ x 1 3/4″) finished size
bear the date the photo was taken (not the date the photo was printed) directly on the back (stick-on labels are not acceptable).
