Please answer my following questions

1. Paragraph 6 (List all names that you have used) :
I have never changed my name, so do I have to leave it blank or write my name and family name ??

2. How many copies of my current passport, do I need to submit ?

2. You only need one copy of the biographical page

Please explain what are biographical pages ??
Also please tell I filled my Physical presence Calculator giving the date of signatures as September 13 thus physical presence calculator counted my eligibility period from September 13, 2013 to September 13, 2018. When I sign the citizenship application, it says make sure you are eligible one day before the signing the application date. I am eligible on September 12 too, but by eligibility is till September 13 as per calculator
Is it OK É or do I need to amend the Physical presence calculator to September 12
