Questions about filling in BOWP application form

Hi All,

My PGWP expires on Jul 9 2016. I submitted my e-APR on Jun 1 2016. I am trying to applying for BOWP on paper basis, have some questions when I fill in IMM5710, hope someone could help. Thank you.

1. I am applying for one or more of the following: a. Work permit with the same employer, b. an initial work permit or a work permit with a new employer, c. restoration of your status as a worker, d. temporary resident permit.
- should I select A??? Do i need to select D as well??? I am currently employed as a full-time, do i need to apply for temporary resident permit now? I have no intention to leave Canada before getting my PR.

2.what type of work permit are you applying for?
- should i select "open work permit"?

3.Have you been issued a certificate under the Provincial Nominee Program.
- should I select "Yes"? I get the nomination under OINP - Human Capital stream. Are there any differences between "Provincial Nominee Program" and "OINP - Human Capital stream"? i heard one type of PNP can apply BOWP directly without AOR (received after submitting e-APR), but OINP - Human Capital stream cannot do that.

4. Give details of your employment for the past 10 years.
- should I list my off-campus work experience? There are only 3 spaces. But i have 5 different workplaces including my off-campus work, where should i list the rest of two?

5.Have you previously applied to enter or remain in Canada?
-should i list my application for study permit and pgwp?

1. Select A. In fact this question is not relevant for an Open Work Permit. No need to select D.
2. Yes. OWP.
3. No, as you're applying for PR through EE, and not through Provincial Nomination (PNP). http://www.cic.gc.ca/english/work/permit/extend/permanent.asp
4. You can attach excel or word document. There should be space for attachment of additional document. As I remember there is only one space, so I put all information I wanted to attach into single word file.
5. Should select "Yes" and list it in short.

A couple more questions about filling out the BOWP application form. I am in a bit of a unique situation as I currently work in Gatineau, QC but live in Ottawa, ON. The BOWP Application asks for the address of the current employer and address of the intended employer. Those two addresses are both in Quebec for me, but when I add my employer's QC address, the form requests that I include a Quebec Acceptance Certificate (CAQ) number and expiry date. I don't have a CAQ. What should I do here? I can perhaps request that my employer transfer me to an Ontario office and include that in an explanation letter? Any thoughts?

As it was a BOWP and i didn't consider those necessary/relevant, I didn't fill anything for intended work (also i was planning to and have since left that job) also to ensure they don't give me a closed work permit by any chance, I don't see why this info would be relevant for a BOWP. Anyway i haven't gotten mine yet so hopefully they won't have any issue

Also the intended work part asks for LMIA and as i was not applying through employer or with LMIA i figured not needed
