This is how I filled in my EE questionnaire after I get the invitation.
1. How many family members does Emily have? (This includes Emily, a spouse or partner, dependent children, and their dependent children.) – 1 (myself only)
2. Does Emily have a relative who is a citizen or permanent resident of Canada? (The relative must be 18 or older and living in Canada.) – Yes, my brother
3. Please tell us about Emily’s immediate family members that have not been identified in the application. This includes any parent, any spouse or common-law partner, any child including adult children, and any brothers or sisters. – I listed my parents and my sisters, who are recently living in my home country and have to intention to come to Canada

My question is how I should prepare the document required in my personal checklist.
1. Proof of Relationship (You must provide proof of your relationship with your host or family member. This can include: a marriage certificate Statutory Declaration of Common-Law Union (IMM5409),birth certificate, official document naming you as a parent ,copy of the inside back cover of the inviter’s passport showing the inviter’s parents, if applicable)
Question: What should I attach here? I don’t have any family members included in my application, just myself, what relationship should I proof??? My relative’s (which is my brother who lives in Canada) birth certificate and my birth certificate????
2. Family Member Proof of Status (You must provide a copy of your family member’s Canadian Citizenship or Canadian immigration status document (e.g. Canadian passport, Permanent Resident Card, Study Permit, or Work Permit). Please provide a copy of both sides of the document)
Question: What are the family member included here? My brother, my parents, and my sisters? My brother is PR here, I can provide PR card. How about my parents and my sisters living in my home country?

Another question regarding to my work experience.
My case is I did part-time job when I was in school, and I started to work full time under NOC D after I graduated, and now I am working under NOC A. All my work experience is in Canada. There are two sections in EE questionnaire, Work History and Personal history.
1. Work History Section: Should I list my full time working experience NOC D and part time job I did when I was in School in Work History Section, or just NOC A work experience that helps me get score under EE system?
2. Personal History Section: System asks for no gap after age 18. I did part-time job when I was in school, should I list two rows (Education and Employment) for that period? Or just one row for my education?
3. In checklist – Employment Records: Only work experience which helps applicant to score is required, or all work experience including part-time working during school?

;) Bonus question: Lots of ppl mentioned they are requested to submit schedule 4/4A after submitting e-APR, can I submit it with my e-APR in part of Letter of Explanation?

I cannot believe you are reading this line, a big applause to you for your patience, and thank you very much for giving me your suggestion and advice. :)

1. Just for your brother.

2. Just for your brother again.

3. Just NOC A.

4. Just 1 row for Education.

5. Just what is required to score points.

6. No harm but not needed. Schedule 4 is required only for PNP applicants.

Thank you very much, Asivad Anac. Just want to confirm with you, applicant for OINP-Human Capital Stream doesn't count for PNP applicant, right? I got the additional 600 points for Provincial Nomination - OINP Human Capital Stream.

Does count as PNP. You can choose to provide Schedule 4 upfront.

Thank you! I will attach schedule 4 with my letter of explanation, hope that could save some processing time. :)

One more question, the questionnaire asks "Has Emily received a nomination certificate from a province or territory?"
- should i choose yes or no, i got the 600 points from OINP Human Capital, but I don't have nomination certificate, the certificate should be a piece of paper given by Province, right?

Thank you!

Yes, OINP sends the physical copy of the nomination certificate via Purolator.

Yes, OINP sends the physical copy of the nomination certificate via Purolator within a week of intimation of nomination. I believe you would have received yours by now.
