Tell where the form is reviewed to seek asylum through the Canadian Embassy or Mazma Refugees

What I assume ,you are asking for RSD Registration by UNHCR to be eligible for asylum ?Are you ?
If so you will get the forms form nearest UNHCR office and they will help you to fill it out.Once you are approved ,you may seek asylum for suitable country through UNHCR
If this is not the case then brother you have to explain in detail
