Dear Friends. I have my citizenship ceremony tomorrow. I was reading the preparation instructions on the below link in the "What to bring" section and it says to bring "a signed copy of the Permission Release and Consent form (included with the notice)"
Does anyone know what form is being referred to here? This is not included in the email notice I received regarding the citizenship ceremony. The only form that is included in my email notice is "Oath or Affirmation of Citizenship form" which I dont think is the same thing.
