Question regarding filling out forms!

Sorry if this is a bit of a sill question,

Im just wondering for personal history and history of where me and my partner have been living do the dates have to match up like for instance

Address 1: 09/2012 - 12/2012
Address 2: 12/2012 - 09/2013

Or should I do;

Address 1: 09/2012-12/2012
Address 2: 01/2013-09/2013

Oh and also for addresses and such when Im putting my present address do I fill in the "to which date" box?
As I am currently living there and have no plans to move!

I believe that the format is yyyy-mm, so you would write:

2013-09 to [date that you send the application]
2012-12 to 09-2013
2012-09 to 2012-12

Doing it the other way (option 2 in your post) could lead CIC to believe that the month of December, 2012 is missing, since
they aren't asking for days, only month.

I hope I'm right because that's how I listed my dates. :-\

I was unemployed for a while but did some temp work (for about 7 days) should I include this in my application?

Yes, be honest, even if it was just temp work. Make sure you don't leave any gaps in your personal history.
