Question regarding Consent for minor

Hello,

I am planning to apply for citizenship with my 17years old daughter. I have full custody and her father will not apply. Do I need to have sort of consent form like I did for PR?

I can't find any form to fill or instruction regarding this.

Any advice would be appreciated.
