You can write a request letter with self addressed envelope and they will send you the form by mail. Ako nagparenew sa friend ko sa main office sa Manila last year. Nagpadala ako ng photos tas authorization letter. Narenew naman pero wala akong thumb mark at ang NBI clearance ko e sa CEM-Manila pinasa kasama ng passport naming mag ina.
