Hi all!

I adopted a western name to use in school and at work, instead of using my legal name which is hard to pronounce. When I graduated only my preferred name was printed on the degree.

Therefore, as I was applying under Express Entry FSW, I answered “Yes” to the question “have you used other names?” and now have to provide a Legal Name Change form. In my case, I need to prove that I am the person who received the degree. I’m not sure what to submit since I never changed my name officially.

Any advice? I’ve read other threads but they seem to be either a name change due to marriage or a nickname used informally. Some ways I can think of:

1. An explanation letter about how I adopted the name, with other official letters from the school showing that I indeed received the degree.
2. Other documents - in the Employment Records section, my employer did mention both my preferred and legal names.
3. Notarized affidavit - Not sure if this is an overkill if I have the material above already. If anyone has done it, please advise:)

you need to submit a AFFIDAVIT in which you mention both names belongs to same person(final name you are using now). and explain your situation in LOE.
