Question for medical exam form, sent straight to CIC?

Hi guys,
I'm sponsoring my husband and we have everything together except for the IMM1017. When he had the medical check the doctor said the form is sent straight to CIC but my checklist indicates that we need to include a copy (which we now don't have). What should we do? Go back to the doctors office and have him refill it out or trust that a copy does get sent from his office to CIC?

hrmm... from my understanding the form the doctors office gives you once you completed the exam is how they find your results. The results go directly to the CIC but the form that is given to you to send with your app is a carbon copy of a form they attach to the exam results. I would call maybe someone made a mistake? or was new? GL!

Yes the medical form is sent straight directly to CIC, but I believe we included something like the receipt for that medical exam. I'd have to check on what we included my spouse looked after that part of it, but for sure they would not provide us a copy. You also provided the medical examiner with pics right, they submit those too and verify that the person examined was the person in the photographs.

I am applying outland and I was given an Appendix C it was a triplicate carbon copy form 3 of the 4 pictures we stapled on to each copy and the final photo onto a bigger brown envelope that the nurse told me was for my x-rays. one of the 3 copies was given to me once my exam was complete and it was included with the app. hope the link helps....

Up-front medical exams and forms (IMM 1017 EFC and Appendix C)

Up-front medical exams are performed when applicants arrive at their immigration medical exam appointment with a completed Appendix C – Medical Instructions form.

The Medical Report: Section A (IMM 1017 EFC) is used for up-front medical exams. DMPs should have a supply of IMM 1017 EFC forms on hand. To order forms, see the instructions in the table above.
