Question about US rejections

I have been accepted to a good university for Master's program in Canada. I have two questions:

1. I have been denied by the US consulate three times. In the Canadian visa application form, Background information section, question 2: Have you ever been refused any kind of visa, admission, or been order to leave Canada or any other countries,
Should I just list the three times I was rejected or do I also have to provide the reasons of rejection?

2. Question 2, should I just provide the copy of my most recent passport or also the copy of the old passport which contains all the stamps and visas?

1.It will be a very good idea to show your refusals in canadian study permit application. But you do not have to write that you got 3 times refused. Just write that you got refused previously by US. When they will look up your application they will be able to see your previous refusals as US and Canada high commission share their data.
2.You just need to send them your most recent one.

Thank you for your response Andy Singh. Are you sure that just mentioning that I had US rejections is enough? The question says provide details.

provide details: "REfused educational visa for the United States, 9/2010, 12/2010, 5/2011. REasons were . . . " If the reasons weren't given, say so.
