I am planning to apply for citizenship because I have earned the 1,095 days to be eligible and now stuck in the Section 11 as I have worked a few part time jobs.

Do I have to put them all or no? I am still working full time on my main job and no gaps with dates.

1. small resto - 2 months
2. coat check - 2 days
3. sushi store - 2 to 3 months?
4. store in food court - more than 4 months?

I don't want to put them as I don't know what to put in the Name of Contact/Phone number/Email address because 2 of them are closed and no ways to contact them for their full names and number.
Other is I don't even remember the owner name which is embarrassing but that was two years ago.

I just want to know if its ok to omit these on the citizenship form. Hope that someone can help me.

No its not ok to skip all. Its better to put them all and in contact info put "N/A" if you don't have their contact info or if you wish not to include there name in your application.

Correct, if you don't know, simply write N/A. I provided the general number I could find on google for previous employers, and no email etc for chain restaurants I worked at.
Anywhere you worked and got legally paid for will have given you an RoE and a T4, so they will verify the employment with the CRA anyway. If they contact the employer, employer will simply verify that they have employed you and provide the details if necessary.

Thank you so much! How about the Police Certificate? I have one from the country I came from but its already expired, can I still use it or do I have to get a new one?

Did you spend more than 183 days in any country during your eligibility period? Here is the language in the application guide. That should help:

Within the last 4 years, if you spent 183 days or more in another country (other than Canada), you must provide a police certificate. You must provide a police certificate for each country where you spent 183 days or more. If you cannot get a police certificate, tell us why.
Example 1
In the past 4 years, you took one (1) trip to France that lasted 200 days. You would answer “Yes” to the question and you would need to provide a police certificate from France.
Example 2
In the past 4 years, you took 10 trips to the United States of America (USA). Each trip lasted 3 weeks, for a total of 210 days. You would answer “Yes” to the question and you would need to provide a police certificate from the USA.
Example 3
In the past 4 years, you spent one year (365 days) working in Singapore. While working in Singapore, you took a trip to Malaysia (10 days) and Thailand (10 days). You would answer “Yes” to the question and you would need to provide a police certificate from Singapore. You would not need to provide police certificates from Malaysia or Thailand.
Example 4
In the past 4 years, you took one (1) trip to Europe where you visited Portugal (5 days), Spain (7 days), France (10 days), Belgium (3 days), Netherlands (3 days), Germany (21 days), Switzerland (7 days) and Italy (21 days). You took a second trip to Europe where you visited Ireland (14 days), Scotland (14 days) and England (21 days). You went to Germany for a business trip that lasted 60 days. The total time you were outside of Canada was 186 days but you were not in a single country for 183 days or more. You would answer “No” to the question and you would not need to provide police certificates from any of the countries.

Yes, I did spent 180 days in that country. I got my PCC for April 2015 and landed in Canada on July 2015 so I was wondering if I can use it or not.

In that case I would say no you can't use an expired cert. The certs only last for a certain amount of time so I would apply for a new one.
 
In section 11, the part " where were you physically located", do I put my home address or my job/ university address?
