Help! I'm so confused! On this website (www.cic.gc.ca/english/resources/publications/dmp-handbook/section-11.asp) it says "This form is usually sent to applicants by the immigration office."
But...I'm also reading that the doctors office should have these forms...
I live in Ottawa, ON, so should the doctors here have these new forms or do I have to order it? The receptionist was useless when I asked her...she also said I needed a Client Number which the Immigration office provides, with 4 photos attached...huh? We haven't applied for anything yet so why would I have client number??
