question about fill form IMM5409E

I am an international student. Now I am prepareing for applying work permit. I checked online, one of the form I have to fill is IMM5409E (statutory declaration of common-law union). But my wife is staying in my country, not here in Canada. I don't know how to fill the form. Hopefully I could get suggestion from you!Thank you so much!
