Yes, You are right to worry. 99% it is rejection. And most probably it will be due to in sufficient job duties.
Now you can waste your time in worry . A better way is to cross check your work duties with NOC outlined duties. If they are almost same then you should not worry because then your reconsideration case will be strong.

1% chance is that it will be system problem.
