The problem with having the medical done in Canada, and having the application processed in Hong Kong is that HK would not normally be requesting medical results from Ottawa. If you feel confident about getting the message to HK to request your results from Ottawa, it might not be an issue . . . but it's a complication you might not want. You might also try to get the DMP to re-send your results to the
Regional Medical Center that deals with HK.

You do not
have to have a representative in HK - your application will go first to the Case Processing Centre in Mississauga, ON for processing of your sponsor. CPC-M transfers the file to the overseas visa office depending on information that's entered on the sponsor's application form IMM1344A, Section 6A-C on page 3. If you were admitted to Canada for at least one year with your original entry document, you can use your Canadian address as your residential address,
and you can elect to have an outland application processed through Buffalo (that would solve the issue of the medical results). If you
want the application processed through HK, regardless of an option to be processed through Buffalo, you indicate that by answering "No" to Q#6C. It will make more sense when you look at the form.
However, if you were
not legally admitted to Canada on entry for at least one year, you would indicate that you want the ap processed through HK by using a residential address in that country on that same 1344A form, Q#6. They will only use that address to determine which overseas visa office to send your file to - you can always use your Canadian address as your mailing address so that all correspondence comes to you in Canada.

If your application is processed at the overseas office in HK, you will have to attend any required interview there - and you will have to submit your passport to that visa office to get your Certificate of Permanent Residence. (Some visa offices request the passport during the process - before they're ready to finalize - and you might not want to be in Canada without your passport, so that's a consideration, too.) Only inland applications have interviews within Canada, and only inland aps having landing interviews within Canada.

Your sponsor needs to sign an undertaking promising to provide support for you for three years - but there is no minimum income requirement for spousal/common-law sponsorships.

HK is currently finalizing sponsorship applications in 5-8 months, plus add another 4-6 weeks for processing of your sponsor at CPC-Mississauga. (Buffalo finalizes in 4-9 months.)

It is possible to contact agents at the Call Centre for more information, but you've found a pretty good source of information here. Most people report that they get different answers to the same questions when they contact the Call Centre - depending on which agent they talk to.
