question about Background declaration IMM-5669

Hi folks

I'm currently living in my country of residence (Qatar) since January 2007 in a working visa , but i came there two times before in 2006 in a visit visa looking for a job, last one was is 11/2006 i stayed for 2 months i found a job then i left for 2 weeks and came back in a working visa.
The question is:
should i mention that in Question-8 (personal history) in Background declaration IMM-5669 and what should i write in that last column (name of company, employer..etc) , and in the Supplementary Information-Your travels since it was not by then my country of residence??

you have to mention about your travel, study, work or unemployed if any of these.

so i should write about the 2 trips in both forms
one more question regarding the addresses (question 12) i should put the address of any long trip -say 3 weeks or more- right?
