Hi Everyone,

I worked directly under a doctor on two separate occasions over the past 4 years. The first was a full one year stretch and the second was for about 5 months. Both times I was hired as an independent contractor and filed my taxes as self-employed accordingly.

Would stating the name of the office and then in brackets putting "Ind. Contractor" be sufficient or should I be attaching a separate letter to explain my job history in detail?

At no time did I ever leave the country for work. My work always took place at the same office every day.

Finally, is proof of employment only needed for those working outside of Canada? I just want to make sure that I don't need to provide any supporting documentation if my jobs were all within Canada. Thanks!

Did you incorporate? Does your corporation have a GST tax ID with CRA? Did your clients make payments to the order of your corporation? If these are true, then you were employed by your corporation and you should input your corporate name as your employer.

No, I did not incorporate. I do have a Business Number and paid HST during the 2nd self-employment stint. I would invoice the doctors and they would pay me.

You had an HST number with CRA? Then you were employed by the name of the "entity" registered with the CRA. If the "entity" is simply your name, then I don't know the correct answer.

Yes, the business name is just my name. Guess I'll be on the phone with CIC again lol. Worst case scenario I'm sure I could just attach a letter explaining everything to be safe.
