Question 11 - Educational Institute Information

Under the "Name of Contact" section for my educational institute I have written either Registrar's office or Principal's Office and given the phone numbers for these offices. Will this suffice?

Also in terms of formatting I have split up my education and work since there is significant overlap. Is this OK?

Yes, it works.
For the formatting, the only thing is to make sure you cover the whole 5 years period. But I'd say better go with latest to newest or the opposite instead of grouping by type, overlap or no overlap.
