So, this is a bit complicated.

For about 2 months, I was transitioning from one location for the same job to another location. I was employed by the same company, but I was taking time off to look after a family member outside of canada. In section 10, I wrote down a couple of addresses I stayed at within those two months.
But in section 11, I covered that period with employment - company and the location i was employed at. I don't know if this could be confusing for them?

You can always attach a cover letter explaining your situation on detail .

I am a bit confused reading your post as well. During the two months periods, did you take time off to take care of family member as extended leave? Were you outside of Canada?
If so, you are still employed with that company as you stated in question 11. Here you can also list the same company twice, each line with address of the two locations that you were transitioning between during the two months if that's possible.

As xylene suggested, a cover letter can be included to further explain your situation.

Because I was taking care of a family member, I did receive EI. Do I have to share supporting documentation for the extended leave of absence?
