Query about application for canadian citizenship-minor

I am in mid of filling up application for my child (below 14 years.)

I am confused about two things in the form.

1) On page 1 , field 5 has written "For official use only" next to it.
So does that mean that I should be leaving these field blanks?
But the instructions for the form asks to fill up.

2) On last page they have Child's signature form , I believe in my case I can leave it blank as my child is below 14 years.

Hello, I applied for my sister who is under 14 yrs old, hope my answers help

1- The "For official use only" is related to the grey box not the section 5 question. I filled up 5 a and b (c and d if applicable in your case)
2- Only the parent signs for kids under 14 yrs old (top box), along with the city and the date

I filled out my canadian citizenship application when i print it in column 12 a my SIN number shows only 6 number how to fix it
