I just received an email requesting an updated Schedule A Background Declaration form (IMM 5669).
I assume this is normal. Has anyone sent their updated form back to them by email (pdf scan)?
 
I am guessing this is some form of a backgroud/criminal check ?

I remember for this one, I filled this out at Korean embassy in Montreal and sent it to a korean police office for their approval. And then they sent it back to me.
 
No, what you mean is the police clearance. I already did this before submitting my application. What they are requesting is an updated copy of this form: http://www.cic.gc.ca/english/pdf/kits/forms/imm5669E.pdf

Check this form properly, May it have some mistake.

Med instructions - 17 Aug
Medicals received - 10 Sep
Passport request - 17 Sep
Payment of RPRF - 23 Sep
PP submission - 11 Oct
PP returned w/ visa - 26 Oct

My Application Status states Decision Made. But when I click on the details, the info provided is only up to 17 September 2015. No updates after that. By the way, my Visa Office is Abu Dhabi.

is it normal to send med and in the same month they request the passports?

I am not sure how normal this occurence is. I was fortunate that our PP was requested immediately after the med results were submitted by the physician. The visa expiry date is given based on the receipt of successful med results. So, i would assume that if the med results are fine, the request for PP submission will soon follow, since time is already ticking for the validity of the visa.

how many months you have before the visa expires ?

Our visa is valid for 12months. Starting from the date that they received our medical results.

Hi Marlee,
I got the same request IMM5669, CIC agent was specific to ask for an updated Schedule A from the date we submitted the application until present and they asked to print , sign it and send it by mail / courier to CIO.. along with Police certificates and receipt of the RPRF payment.

What does it say exactly? For me it says:

"Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please pay a special attention to question 11 and 12. If question 11 does not apply to you please write N/A. Question 8 and 12 must cover 10 years. This must be received at this office by: 2015/12/13"

I checked the form I originally sent to them. I put in "NONE" for question 11. Concerning question 8 and 12, it clearly stated "for the last ten years or since you turned 18, whichever is more recent". For question 8, I started with September 2005 and for question 12, I started with June 2005 (each dates where I moved/changed schools). I understand that when I filled out the documents one year ago, I was therefore only giving them information about the last 9 years (2005-2014) but now another year has passed, which means that the 10 years are complete AND I turned 18 in late 2007 so why would that even be relevant? I don't get it.
