This is the format for experience letter. I thought this format should cover the requirements of MICC.

I know it is some times tough to get letter in format you need but try harder. The employer cannot deny this legally, they just try to play tough so that the dont get these request from all.
Now for accomplishments, it could be any award you won, any significant project you executed or even some project you were part of. I learnt that " Full Time Direct Employee" is must in letters. In canada some jobs are temporary and some fulltime direct. we dont have such systems in our country but they want to see this term in experience letter. Please dont forget that. Its a good practice to attach the business card of the issuing authority. This gives sends positive vibe. Also try to get the original copy. This is not your birth certficate which is difficult to get.
