ITR-V is the suggested format for tax returns by MIDI.I bet that would not have been the issue.Please confirm with her what other documents they requested from her.
This was in the email I received from MIDI

Note: If you are unable to provide proof of social security contributions or proof of exemption from payment of social security premiums, you must provide the original or certified true copy of the original proof of payment of the income tax.
Warning

Documents issued by an employer and electronic documents are not accepted as proof of social security contributions or the payment of income tax.

i also dont know what is the problem i think ITR-V WHICH SHE SENT is electronic document, ok tell me can she file administrative review..

She can go for administrative review,if there is an error made by MIDI itself ,like wrong calculation of points etc.But you can give a try..

Finally the requested documents reached Montreal Yesterday..I was asked to submit documents on August 8th ,I was able to send them only by this month..It's very hard to get all the documents in one place...Let me see what's gonna happen next..

Can i ask what's the actual document did you submit for the tax form with your work from UK when you sent them the update? I hope your interview will be waived so you can have your CSQ soon

Jelnaby, the documents they requested to update on October 02, 2014 from me reached montreal on the 15th of October 2014, we have almost the same date.. Lets hope for the best result.

I APPLIED IN QSW AS A NURSE. I DIDNOT FILL "DECLARATION BY A CANDIDATE PRACTISING A REGULATED PROFESSION OR TRADE IN QUEBEC FORM NUMBER A-0527-JA" FORM. WILL IT AFFECT IN PROCESING? Can I send it again? I sent my documents in july 2014. Will my form refurn back because of this? I will be grateful for your suggestions and experience.

I got AOR recently. But before getting AOR I sent the missing form with cover letter. I don't know whether that form is included in my folder or not.

What shall I do in this situation ? Shall I resend the form with cover leter or wait till doc request? Thank you in advance.

From HM Revenue & Customs I requested tax statements as well as employment history statement.Because tax statements shows only the tax ,but employment history shows the tax year along with the employer details.So now I have submitted both as well as P60 from employer

What is the subject of the letter you receive? Please see below for administrative review.

"A letter issued by the Ministère informing you of an intention to refuse, an intention to reject, a decision to reject, the closing of a file or the return of an incomplete application for a selection certificate cannot be the object of an administrative review."
