I just read the link...it says November 2012...however, my fiance didn't get these new forms, he just received one IMM1017. Maybe they made a mistake with the date or this only affects how the physician sends the medical results.

yess these forms are not for the client to bother with.the doctor will check it and upload it.just she toke as well her 1017 forms as well and they didnt need any photos.ur fiance surely has been pictured in the doctor office.

He didn't get them yes but they did use the e-medical step rather than the paper-based one, which explains why he wasn't provided with a tracking number for the medical

exactly same to my friend case.

I am not sure if the doctor took his pictures, because he took 4 small pictures with him since that's what was indicated on the form.

no they wont use the photos in emedical.it is a request that the picture to be taken inside clinic as precaution for identity concern.this is what the doctor say to my friend

In the end, he did his medical and it has been received fast and soon he'll be hearing good news, that's what you're mostly looking forward to I'm sure :)
