I have received my medical form but the name on application is misspelt and also the country of citizenship is wrong.How do I rectify that.Please advice

No issues.
U could ask ur DMP to rectify those forms, by Xchecking ur pps.

thank you also I hv to obtain pcc from dubai & ksa from dubai.Can I go to the respective consulate in USA and request the same because I do not have anyone who can help me in this regard.
