Q regarding 'Extend Stay/Change Conditions' form

Hello,

I'm looking for clarification regarding the open work permit application, 'Extend Stay/Change Conditions', that my husband is including in our spousal Permanent Residency Sponsorship application (which we're sending in a few days). His temporary visa runs out on Aug 12. I'm wondering if this form is all we need in order to request an extension for him OR if we need to separately apply with a different form. (The reason for the 'Extend Stay...' form is so he can legally work while we await permanent residency, not extension).

I looked through some other threads and finally found the answer I was looking for.
