Just wondering too if you guys use the surname of your husband "family name" when filling out the form? or did you use your maiden name? i already use my husband's surname on my passport, so i guess i could use it when filling out the form when it says, "family name"?

On the forms it does require you to write your name exactly as it is on your passport. Then there is another part where you can write other names you have used (maiden name, nicknames etc). We have finished filling up forms and we are now waiting for my police clearance certificate from my home country.
