proof of medical examination

I'm filing the sponsorship application outside of Canada.
For the proof of medical examination for the sponsored person, the checklist asked us send Copy 1 of Medical Report: Client Biodata and Summary (IMM 1017) However, the doctor in the clinic says they don't issue that. In the end we only got a piece of receipt from the clinic.

Can we send the receipt for the proof of the medical examination? If yes, do we have to send the original one or just the photocopy of the receipt?

If the doctor was e-medical enabled, all you need to send is the IME # ("Immigration Medical Examination Number") and CIC can match up the medicals with the results.
