Proof of Medical Exam

My husband is applying for Permanent Residency from within Canada.

He has just undergone the medical exam. I asked the physician if she was supposed to give us a form to include in our application as proof the medical exam was completed. She said no.

I reviewed the Document Checklist when I got home which gave the unspecific requirement of 'Proof of medical examination'. Looking at the instruction guide also on CIC's website, it said "You will need to include in your application proof (Copy 1 of medical Report: Client Biodata and Summary IMM1017) provided by the Panel Physician."

Currently, in regards to the medical exam, what am I required to include in the application? The doctor offered to dictate a letter stating she completed the exam for my husband.

You need two pieces of documentation; the IMM form and a document from an authorized panel physician. There is a list on the CIC of panel physicians by country.

How much did you all end up paying for this medical exam?

I called up the only one in my state and he wanted to charge $420 for all of it! I have medical insurance in the USA that covers regular physicals and most things, anyone know if there is any way to get insurance to cover a medical exam? I am considering possibly driving to a different state to get this whole thing for cheaper as it costs almost as much as half the entire process! Where did you get your panel physician medical examination done?

The doctor had to be on the list of approved doctors, and YES, s/he was supposed to give you a copy of the form to include in your package to be sent. You need to go back there with your paperwork and show him/her otherwise when you send in your package, it will be held up b/c you're missing documentation. That's the #1 factor in increasing your wait time for processing (missing docs). I met someone that works as a Visa Officer and that's what she told me!

Because the closest panel physician was outside the 400 km radius the website stated, I called CIC and arranged for a local doctor to do the exam. You provide CIC with the doctor's details, they decide if they are qualified and then they send the doctor instructions/forms. The doctor that conducted the exam is not on the list of Panel Physicians, but was approved by CIC as qualified to do my husband's examination.

Only the Document Checklist [IMM 5491] for applicants living outside of Canada has Proof of medical examination for the sponsored person and each of his or her dependants (Copy 1 of Medical Report: Client Biodata and Summary (IMM 1017), signed and provided by the panel physician) listed as a requirement whereas Document Checklist Spouse or Common-Law Partner in Canada (IMM 5443E) simply states Proof of medical examination (if you do not hold a valid Medical Report, instructions will be given to you when you receive your notice of eligibility) as a requirement.


Again, we are applying from WITHIN Canada.
