send them your parental leave payment information print out from service Canada website. or ask service Canada to send you copy of that. also send them t4e

Dear just a quick queztion if u don't mind , I went to the web site , it shows me a blank form of t4e , do I print and fill it or ask cra to send me stamped and filled form , thanks

No, because...: "A T4E slip is issued to everyone who received EI benefits or repaid an overpayment in the previous year, for example, from January 1st, 2013 to December 31st, 2013. This slip must be filed with your income tax return for that tax year." It's like any other T4 slip you receive by another institution to declare income you received from that institution.
