How to show proof of employment?

Is there a specific format of the document and what are the main things that needs to be in the document?

Proof of employment requirements:

1. must be on company's official letterhead having contact details and address of the company.
2. must have your name, designation, date of joining, date of relieving ( if previous job).
3. must mention your key duties at the job ( must be similar to primary NOC you selected)
4. must mention full-time/part-time/permanent/regular/contract as well as number of hours per week.
5. must have your salary details.
6. must be signed by your supervisor/ manager/ immediate head/ boss/ or HR official.

Thanks for reply, What if i work for an employer and work at a client location, do i need to get a letter from client?
