Prohibitions under the Citizenship Act(CIT 0039)

Hello

Today I got test invite letter, It has a form "Prohibitions under the Citizenship Act(CIT 0039)", my question is do I fill it and sign it at home or in front of the interview officer? and

it ask about 1. File no and 2. Client ID

so what to write in these 2 places ? please forgive my ignorance ,

You better sign in front of the CO, or aleast confirm in front of him that you have no prohibition, if already signed.

I did my test two days ago. I received my invitation for the test ONLINE but it did not have this paper.

However, they did distribute it there and we had to sign it in front of officer. It's a new paper we have to sign so not many of us received it by mail/email.

I signed it in front of the officer, I guess you should do the same.

The client ID is the 8 digit number printed at the front of PR card in the format xxxx-xxxx
File # would be printed on your test letter or on your AOR usually starting with C0000xxxxx

Got two more invitation letter for the test day on Friday. So in total 3 ???. For exact same days hours but for the last two had attached what you are speaking about the form Prohibitions under the Citizenship .......wondering why????? With the submitted application we already sign it.
Did you say they asked everybody onemore time on the test date to sign this form? May I ask where did you write the test?

Did they said anything when do you have to take your OATH?
