Both can take more time. Longer processing time doesn't indicate whether you are going to be approved or refused.

Preliminary check whether application is complete---if passed---->Review of documents by visa officer----if passed---->Application is sent to an agency for background checks and a medical request is sent if not done yet----if passed----> Passport Request

Incase of failure

Preliminary check whether application is complete---if fail---> request for more documents or rejection

Review of documents by visa officer----if fail----> rejection

Review of documents by visa officer---if fail----> rejection

The longer the wait means the more steps you have passed. Most rejections happen in the second step, which is why rejections are faster than approvals.

In some cases, there may be a problem in one step and delay your application and still be a rejection. Usually this is not the case.

This theory is based on the Abu Dhabi Embassy and should apply to many others.

Go by the processing time updated on CIC website every week...if your processing time has passed by the ideal time, then CIC is awaiting additional security checks, background checks and authentication of documents.
If upfront medical is not done, then they are awaiting medical reports.
Ideally refusal and approval takes same time as every nationality has different processing time and you should look for your national processing time.
If refusal case is very strong like medically unfit/criminal record and forged documents submitted then its usually delayed as CIC updates A40 and A44 sections in database, which means candidate is prohibited from applying to Canada.
Be patient for atleast five weeks latest.

My all documents are authentic and medical is also ok. So expecting positive result.
