I am wondering if anyone can help. Is there a defined period for which we have to mention previous visa refusals? if someone had a refusal 20 years ago, is that required to be mentioned?

You need to list all refusals regardless when they happened. There is no defined period. Refusals from 20 years ago need to be listed.
